IMPORTANT: GRCS Health Screening Tool Updates

Dear Grand Rapids Christian Schools Parents,

I hope you and your family enjoyed the long holiday weekend.

Kent County Health Department (KCHD) informed us today that they are moving the health screening tool into “Phase 2” beginning tomorrow – Thursday, September 9, 2020. The majority of schools in Kent County are now using the screening tool as well!

Here is what Phase 2 means for you:

One Text Per Campus With School-Specific Links

You will receive one text for each campus at which you have students enrolled. For example, if you have three children enrolled at two different campuses, you will receive two texts each day.

If you have children at multiple campuses, please carefully read each text you receive as each text will identify for which GRCS campus the link should be used. The logo for that campus will also appear at the top of the health screen survey.

It is critically important that you only use the link to complete the health screen for your child(ren) who attend the campus referenced in the text and on the survey.

No More Student IDs

Phase 2 does not require that parents input student ID numbers. Beginning tomorrow, you will be asked to:

1. Enter your child’s first and last initial.
This is the first initial of their legal/given name and the first initial of their last name. Please only use capital letters.
*Note: If your child’s last name has two words, you should only enter the initial of the first word. For example, Tom DeJonge would be TD.

2. Select your child’s grade level from the drop-down list.

3. Enter your child’s date of birth.
*Please enter it as MMDDYYYY. Do NOT use spaces, dashes, or slashes. (ex. 09082020)

4. Answer a question about any symptoms your child is currently experiencing.

5. Answer a question about if they have been exposed to someone with a confirmed case of COVID within the last 14 days.

6. Provide authorization.
In order to submit the health screen, you will also need to select “Yes” to give authorization.

No More Reminder Texts

The Phase 2 screening tool will only send one text per campus to each cell number we have on file at 6:00 am each day. No additional reminder texts will be sent.

The survey is only available until 8:30 am each day.

Updating Information and Issues Accessing the Survey

If you are certain that a parent will not ever complete the survey and wants to be removed from the list, please email Loreen Standel and include the name and phone number of the parent who wishes to be removed from the list.

If your cell phone number changes or if you need to change the cell phone number that receives the daily text messages, please email Loreen Standel.

If you are unable to access the survey from the link in the text, please email Loreen Standel.

Thank you for your patience while our schools, as early adopters of this screening tool, worked alongside KCHD throughout the first several weeks of school to refine the process. Thank you, too, for completing this screen daily for each child.
