Preschool

☑️ Child Information Record
To be completed each year your student is enrolled in preschool.
 
☑️ Health Appraisal Form with Immunization Information
To be completed each year for all preschool students.
Families can provide an official copy of their child’s up-to-date immunization record from their child’s doctor or a certified waiver from the Kent County Health Department. If immunizations are documented on the Health Appraisal Form, the front and back must be filled out and the form must be signed by the student’s doctor’s office.

Not a hospital-issued document.
 
☑️ Concussion Awareness Acknowledgment
To be completed once for all new students (new to GRCS).
 

Kindergarten and New Families

New for the 25-26 school year: Please note that an oral health assessment has been added to the Health Appraisal Form.

All Families | Forms Needed As Applicable

☑️ Major Medical Conditions or Life-Threatening Allergies
To be completed every year as needed for students who have an update to their medical condition that has not already been provided through Verification in ParentVUE.
 
☑️ Prescription Medication Authorization
To be completed every year as needed for students for students who require prescription medications that will be carried and/or dispensed at school.
 
☑️ Court Orders
Per GRCS Board policy, parents are required to provide information regarding any custody or visitation restrictions for their child, which will be kept in the child’s CA-60 record. Without documentation, GRCS sends all communication to both parents/legal guardians and grants both education and custody rights. GRCS will abide by all court restrictions as long as a copy of a custody or visitation agreement has been provided.
