☑️ Health Appraisal Form with Immunization Information
Please note that a new Health Appraisal Form is required every year.
Along with the appraisal, you must submit your child's immunization status in one of the following ways:

Doctor's Immunization Record or Health Appraisal form: Provide an official copy of the up-to-date record from your child’s physician or your physician can fill out and sign Section 4 of the Health Appraisal.

Certified Waiver: If your child is not up to date on their immunizations, please contact Kent County Health Department at 616-632-7200 to schedule a certified waiver appointment.

Please note: Not a hospital-issued document. If your child previously attended preschool at GRCS, their certificate is already on file.
Kindergarten and New Students

To be completed every year as needed for students who require prescription medications that will be carried and/or dispensed at school.

☑️ Court Orders
Per GRCS Board policy, parents are required to provide information regarding any custody or visitation restrictions for their child, which will be kept in the child’s CA-60 record. Without documentation, GRCS sends all communication to both parents/legal guardians and grants both education and custody rights. GRCS will abide by all court restrictions as long as a copy of a custody or visitation agreement has been provided.
